Client Bill of Rights and Responsibilities.

AS A CLIENT OF THE DAKOTAH CONSULTING, INC. / DCI PSYCHOLOGICAL SERVICES, YOU HAVE THE FOLLOWING RIGHTS:

● To be treated with courtesy and respect by the staff and all contracted providers. 
● To make recommendations regarding DCI Psychological Services’ rights and 
responsibilities. 
● To receive appropriate care with regards to your health, wellness and well-being. 
● To obtain information about DCI Psychological Services’ services and providers. 
● To receive information about DCI Psychological Services’ clinical guidelines and 
member’s rights and responsibilities. 
● To participate in the planning of your treatment, including the option to consult with 
other personal and professional acquaintances, at your own expense. 
● As a competent member, to refuse treatment. 
● To participate in experimental research, but only when you have provided written, 
informed consent to do so. 
● To be free from mental and physical abuse as defined by law. This includes freedom 
from any act that constitutes assault, sexual exploitation, or criminal sexual conduct. This also includes the intentional and non-therapeutic infliction of physical pain or injury, or any conduct intended to produce mental or emotional distress. 
● To confidential and private behavioral healthcare treatment and the confidentiality of 
your treatment record. This includes your right to approve or refuse the release of information in this record outside the clinic or providers office. 
● To voice complaints about DCI Psychological Services’ or the care that is provided 
according to the provider’s grievance procedure. A copy of the procedure can be obtained from your provider.

AND THE FOLLOWING RESPONSIBILITIES:


● To give to your treatment providers, the information needed in order to care for you.
● To participate, to the degree possible, in understanding your health problems and in 
developing with your provider, mutually agreed treatment goals. 
● To follow the treatment plan and instruction for care that you and the provider have agreed upon.
